HM maintains policies, procedures and safeguards to ensure the protection of your Personal Information. Personal Information of our customers collected by HM is used for the sole purpose of servicing your needs. Changes to HM’s Privacy Policy
Any changes to HM’s Privacy Policy will be communicated as they occur. In addition, HM will provide current customers with a current Privacy Policy annually. Filing a Complaint
In the event you feel your Privacy Rights have been violated with respect to your Personal Health Information you may file a complaint directly with the U.S. Department of Health and Human Services. In addition, complaints for any violations of the privacy of your Personal Information may be submitted directly to HM at the address below.
